Office Setting & Admin

Our office is located within 2 minutes of I-87 Northway Exit 9, in Clifton Park, NY. The setting is a professional office building on a private, wooded lot with parking. Our wonderful Office Manager, Julie, works from a remote office and is available to clients during office hours via Voicemail, Text, Email or the Contact page to help with any administrative concerns.

Online Video Sessions

Clients may schedule appointments for either online video sessions or in-person sessions at their convenience. Online video sessions are available at no additional cost and are accessible via an internet connected computer or smartphone. Health insurance plans typically include coverage for telehealth services for no additional cost/copayment.

Client Portal & Scheduling

Clients use a secure online portal site in order to complete intake forms, manage payment balances, view all available openings and self-schedule appointments. New Clients may request an invitation to the Client Portal via the Contact page. Initial Appointment slots are typically available within four to six weeks. Appointments are available only during weekdays.

Insurance & HSA’s

Miguel E. Diaz, LMHC is an in-network provider with several health insurance plans. If you would like to use your out-of-network insurance benefits, we will submit your claims for partial reimbursement to your insurance company on your behalf. Contact us if you would like us to verify your insurance plan benefits and deductibles for you.

Payments & Fees

The fee rate is $125 per counseling or coaching/consultation session. A $100 fee will be charged for sessions missed without notice (eg. no-shows). A $75 fee will be charged for any sessions canceled or rescheduled with less than 24 hours notice. Payment is due in full at the time of service. You make payments in person with a check, or via the Client Portal.
